Please complete form PCM201 and file this form with the Eaton County Probate Court by E-mail the Eaton County Probate Office or you may drop the form off at the Eaton County Courthouse located at 1045 Independence Blvd., Charlotte, MI 48813. After filling the paperwork, the Judge will review to make sure the situation meets the criteria.
